Pagadian: City Mayor Sammy Co Signed The Executive Order No. 100 Series Of 2021

Pagadian City – “An EO Placing the Entire City of Pagadian Under GENERAL COMMUNITY QUARANTINE Effective November 1-30, 2021 as Recommended by the National IATF Resolution No. 146-A, and Providing Guidelines Befitting its Real Situation in our Continued Fight Against the Spread of COVID-19 Cases”

Important Guidelines:

* Intrazonal Movement of FULLY VACCINATED Senior Citizens within areas under GCQ as provided in paragraphs B & D Resolution no. 124-B issued on July 2, 2021 with STRICT OBSERVANCE of MINIMUM PUBLIC HEALTH STANDARDS shall now be ALLOWED, subject to the presentation of any of the following;

1. A COVID-19 Vaccination Card duly issued by a legitimate vaccination establishment or authority, whether local or foreign;
2. A Vaccination Certification accessed thru VaxCert.ph or issued by the Emergency Operations Center of the City;
3. Certificate of quarantine completion showing the holder’s vaccination status as may be issued by the Bureau of Quarantine.

* TRAVEL PROTOCOLS REQUIREMENTS

1. APOR – travel under official business

(Note: If an APOR travels for REST, his/her travel is considered indispensable.)

Fully Vaccinated APOR

1. Company/Agency Issued ID
2. Vaccination Card/Vaccination Certificate

Partially Vaccinated or Unvaccinated APOR

1. Company/Agency Issued ID
2. Travel Order
3. Resident Non-APOR/Returning Resident

(Note: Submission of the following documents through SPass.)

Fully Vaccinated Resident Non-APOR/Returning Resident

1. Valid ID
2. Vaccination Card/Vaccination Certificate

Partially Vaccinated or Unvaccinated Resident Non-APOR/Returning Resident

1. Valid ID
2. LGU Acceptance
3. Negative RT-PCR or Antigen result (5 days fresh valid result)

III. Non-Resident, Non-APOR

Fully Vaccinated Non-Resident, Non-APOR

1. Valid ID
2. Vaccination Card/Vaccination Certificate

Partially Vaccinated or Unvaccinated Non-Resident, Non-APOR

1. Valid ID
2. Any Proof of Transaction/Appointment from Concerned Government Institution/Private Agency
3. Negative RT-PCR or Antigen Result (5 days fresh valid result.)
4. Returning Overseas Filipinos (ROFs) – OWWA Coordinated
5. Valid ID
6. Negative RT-PCR (5 days fresh valid result)
7. Quarantine Certification issued by the DOH-Bureau of Quarantine
8. Delivery of Good (Note: Maximum of 5 personnel)

Vehicles transporting cargoes/goods are not allowed to carry passengers.

1. Company ID
2. Goods pass/Business Permit
3. Cargo Manifest/Delivery Permit

* CURFEW: 12 MN to 4 AM

* These are the Establishments, Persons, or Activities SHALL NOT BE ALLOWED TO OPERATE, RK, or be UNDERTAKE during GCQ;

1. Entertainment venues with live performers such as KARAOKE BARS, CLUBS, CONCERT HALLS, THEATERS, and CINEMA;
2. Recreational venues such as INTERNET CAFES, BILLIARD HALLS, AMUSEMENT ARCADES, and similar venues;
3. Amusement parks or theme parks, fair/perya, kid amusement industries such as playground, playroom, and kiddie rides;
4. Casinos, cockfighting and operation of cockpit;
5. Outdoor sports courts or venues for contact sports, scrimmages, games of activities.

THIS ORDER SHALL TAKE EFFECT IMMEDIATELY UPTO NOVEMBER 30, 2021.
