Bacolod: BACuna Card Updates & FAQs

Bacolod City – Only outbound travelers going to the following destinations below will be prioritized:

  1. Panay Island
  2. Region 7
  3. Boracay
  4. Manila

Visit RPN DYKB Bacolod

Travelers requesting for Bacuna Card should attach the following in their email:

  1. Proof of Travel (travel ticket/hotel reservation or booking/ Companyโ€™s Travel Certificate)
  2. Vaccine Card
  3. Government Issued ID

  1. VAXCertPh โ€“ certificate issued by Dept. of Health (DOH)

A: Individuals who will be travelling outside the country

  1. Vaccine Certificate โ€“ certificate issued by City Health Office

Q: Who can request?

A: Individuals who completed their vaccination in Bacolod City.

Q: Where to request?

A: City Health Satellite Office: 2nd floor NGC

  1. BACuna Card โ€“ vaccine card with QR code issued by the Government of Bacolod City

Q: Who can request

A: Outbound travelers going to Panay Island, Region 7, Boracay & Manila

Q: Where to request? A:

  1. Vaccine Card โ€“ card issued by a vaccination facility to an individual upon his/her completion of COVID-19 vaccination.
