To further intensify our COVID-19 Contact Tracing the Provincial Government issued Executive Order No. 20-043, s. 2020 mandating for the mandatory usage of QR coded “e-HEALTH Pass” in the entire Province of Surigao del Norte, which can be generated as follows:
1. Register
a.) Kindly visit “https://covid19.ssct.edu.ph/healthform“
b.) Click “Register Local Resident”
c.) Fill out the forms
2. Generate QR Coded “e-HEALTH Pass”
3. Print or Screenshot in your Mobile Phone your assigned e-HEALTH Pass
