ADVISORY TO THE PUBLIC!
For all individuals who want to enter the Province of Surigao del Norte using our EXPRESS LANE beginning on 10 June 2020, you may use our eHEALTH FORM, an online Health Declaration Form, which you can obtain at the link below. By using the generated Health Declaration Form (a sample of which can be seen below), you don’t need to go down from your car or service vehicle everytime you pass through any of our Provincial Checkpoints.

Thru HEALS 911
We will be accepting this June 10, 2020, the printed or image copy of the online Health Declaration Form (HDF) also known as “Health e-Form.” This is a collaborative effort of the Surigao State College of Technology (SSCT) and the Provincial Government of Surigao del Norte through HEALS 911. This Health e-Form will be generated using the link below:

All passengers crossing the Provincial Checkpoints may use this Health e-Form or use the manual forms in filling up the HDF. Here are the following Processes that the checkpoint by-passers may opt to use:
- Provincial entry using the Health e-Form
1. Spraying of hands and/ or vehicle with disinfection liquid upon entry at the checkpoint;
2. Checking of body temperature using the thermal gun scanner. Remember, if your body temperature is 37.5 degree or above, you will be classified as COVID-19 suspected case and will be subject to rapid antibody testing (RAT);
3. Presentation of QR-ready Health e-Form printed copy or image copy and ID together with the documents stated in the Remarks portion of the Health e-Form;
4. After verification, the checkpoint In Charge will allow entry if all documents are in place;
5. For Surigaonons, regardless if categorized as an Authorized Persons Outside Residence (APOR) or not, they will be subjected to rapid antibody testing (RAT) during their entry to Surigao del Norte. The Checkpoint handler or Help Desk Officer will give them the instructions for RAT. - Provincial entry using the manual filling-up of HDF

3. Filling – up of Health Declaration Forms (HDF);
4. Picture Taking;
5. Presentation of ID and other documents;
6. After verification, the checkpoint In Charge will allow entry if all documents are in place;
7. For Surigaonons, regardless if categorized as an Authorized Persons Outside Residence (APOR) or not, they will be subjected to rapid antibody testing (RAT) during their entry to Surigao del Norte. The Checkpoint handler or Help Desk Officer will give them the instructions for RAT.
